Integrating clinical, dermoscopy, and reflectance confocal microscopy findings into correctly identifying a nevoid melanoma

چکیده
NeM: nevoid melanoma RCM: reflectance confocal microcopy INTRODUCTION Nevoid melanoma (NeM) is an uncommon subtype of melanoma that can mimic the morphology of an intradermal nevus both clinically and histopathologically. Similar to other melanoma subtypes, NeM is prone to local recurrence and metastasis, necessitating early diagnosis and prompt management. Recently, reflectance confocal microcopy (RCM) has shown to be a valuable noninvasive imaging device to help differentiate benign from malignant skin lesions with cellular resolution. Here we report a unique case of NeM that clinically mimicked an intradermal nevus. The appearance of the dermoscopic features of irregular globules and polymorphous vessels led to the expansion of the differential diagnosis, which included melanoma, irritated intradermal nevus, and an irritated clonal seborrheic keratosis. RCM imaging was performed to narrow the differential diagnosis and determine the true nature of the lesion. RCM revealed an atypical melanocytic neoplasm highly suspicious for melanoma, prompting a biopsy and averting a delay in diagnosis. This case highlights the importance of RCM as an adjunct tool that adds useful information to the clinical and dermoscopic findings. All of the gathered information (including features seen and features noticeably absent) were used in concert to arrive at the diagnosis of NeM via abductive reasoning or Bayesian inference.

Reflectance confocal microscopy of cutaneous melanoma. Correlation with dermoscopy and histopathology*
In vivo Confocal Microscopy is a method for non-invasive, real-time visualization of microscopic structures and cellular details of the epidermis and dermis, which has a degree of resolution similar to that obtained with histology. We present a case of cutaneous melanoma in which diagnosis was aided by confocal microscopy examination. We also correlate the observed features with the dermoscopic...

Early detection of lesions while minimising the unnecessary removal of benign lesions is the clinical aim in melanoma diagnosis. In this context, several non-invasive diagnostic modalities, such as dermoscopy, total body photography, and reflectance confocal microscopy have emerged in recent years aiming at increasing diagnostic accuracy. The main developments in this field are the integration ...

Skin melanoma is an international public health issue, with a considerable increase in frequency over the past few years. Early diagnosis and excision are essential for good patient prognosis. Over the past two decades dermoscopy has gained significance due to a major improvement in the accuracy of skin melanoma diagnosis in its early stage. However, there are some benign lesions of questionabl...
متن کاملA solitary pink lesion: dermoscopy and RCM features of lichen planus
We present an unusual onset of cutaneous lichen planus (LP) in a middle-aged patient. The initial presentation as solitary, indolent pink lesion required further investigations to rule out malignancy, especially amelanotic melanoma. Dermoscopy and reflectance confocal microscopy findings were found to be helpful in our case in addressing the correct diagnosis.
متن کاملMelanoma and satellite blue papule
The colors that are seen in dermoscopy depend on the anatomic level of the skin at which the chromophores are seen. Blue color can be found in a variety of melanocytic and nonmelanocytic lesions. An 89-year-old man presented with a 3-year history of a slow-growing, hyperpigmented patch located on the distal third of the right arm. Dermoscopy showed an atypical network, irregularly distributed g...
